When Size Matters: Advanced Treatment of Acute Pulmonary Embolism Caused by a Large Snake‐Like Thrombus

Nicola Marchese, Alessandro Russo, Mauro Cassese, Carlo Vigna

ABSTRACT

A 56‐year‐old female patient presented with pulmonary embolism‐related obstructive shock complicated by cardiac arrest. Computed tomography revealed a large snake‐like thrombus. After unsuccessful imaging‐guided transcatheter aspiration, targeting one end of the thrombus, surgical embolectomy was effective in removing a 48 cm single thrombus. A thorough evaluation of pre‐procedural images could help determine the best treatment strategy and should be an essential part of the multidisciplinary decision‐making process.
