Understanding the Integrated Care System for Older Adults with Diabetes
Sun Hee ChoThe elderly segment of the population is rapidly increasing, and both the prevalence and incidence of diabetes among older adults are high. Although self-management is crucial for diabetes treatment, older adults with diabetes often face difficulties in managing their condition due to age-related physiological changes, functional decline, comorbidities, and various psychosocial issues. Therefore, healthcare professionals and diabetes educators must facilitate the effective integration of community resources aligned with evolving policies to improve self-management and quality of life for older adults with diabetes.
