Two-piece magnet-retained silicone lip prosthesis in a mandibulectomy patient with lower lip defect following oncologic resection: A clinical case report

Abstract

Lower lip defects after oncologic resection can severely affect oral competence, speech, esthetics, and psychosocial well-being. Surgical reconstruction alone may not fully restore dynamic perioral function, requiring adjunctive prosthetic rehabilitation. A 58-year-old male with right lower alveolar squamous cell carcinoma underwent mid-segmental mandibulectomy with immediate free fibula flap reconstruction and primary implant placement, followed by radiotherapy. One year later, he presented with partial lower lip deficiency, drooling of saliva, speech difficulty, and esthetic compromise. A two-piece prosthesis was fabricated, combining an implant-retained overdenture and magnet-retained extraoral silicone lip prosthesis. Cobalt–samarium magnets ensured precise retention, while intrinsic and extrinsic pigmentation improved esthetic integration. At 3 months, the prosthesis showed satisfactory retention, stability, and color durability. Patient-reported outcomes Liverpool Oral Rehabilitation Questionnaire version 3 indicated a 47% improvement in overall score, reflecting gains in function and psychosocial well-being.
