Trauma-focused Cognitive Behavioral Therapy for Complex Trauma in a Transitional-age Youth Exposed to Domestic Violence: A Case Report from India

Abstract

This case report explores the feasibility and clinical effectiveness of Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) for a transitional-age adult with a history of witnessing and experiencing domestic violence, emotional neglect, and physical abuse, presenting with symptoms of complex posttraumatic stress disorder (PTSD) and severe depression. A 23-year-old female was evaluated using standardized measures to assess symptom severity, posttraumatic cognitions, and a qualitative interview to identify perceived changes. Nineteen weekly individual TF-CBT sessions were conducted under supervision without parental involvement. Posttreatment assessments revealed significant reductions in PTSD symptoms, depression, and anxiety, along with improvements in psychological well-being, trauma-related beliefs, and self-compassion. The patient reported improved emotion regulation, decreased self-criticism, better boundary-setting with family members, and increased empowerment. These gains were maintained at 1-month follow-up. This case supports the feasibility, adaptability, and clinical effectiveness of TF-CBT for addressing complex trauma in transitional-age youth.
