The South African Flag Sign: An Electrocardiographic Sign for High Lateral Myocardial Infarction

Abstract

The South African flag sign is an acute, dynamic electrocardiographic (ECG) finding typically associated with first diagonal (D1) artery branch of left anterior descending coronary artery occlusion. We report the case of a 62-year-old female patient who presented with a history of chest pain suggestive of acute coronary syndrome, who exhibited this ECG pattern, and subsequently, angiogram revealed a total block of large diagonal branch which was treated with primary angioplasty.
