Background:
Endoscopy is central to gastrointestinal care but is associated with significant occupational stressors, including high workload, physical strain, and psychological impact following complications. Data on endoscopist wellbeing in the Middle East remain limited. This study aimed to evaluate the prevalence of burnout, musculoskeletal symptoms, and emotional consequences of complications among endoscopists in Saudi Arabia.
Methods:
A national cross-sectional survey was conducted between October and November 2025 among licensed endoscopists across Saudi Arabia. The questionnaire assessed demographic characteristics, workload, musculoskeletal symptoms, burnout indicators, complication experience, emotional impact, and institutional support. Data were analyzed using descriptive statistics.
Results:
A total of 76 endoscopists participated. High workload was common, with 73.7% performing ≥20 procedures per week and 78.9% working ≥41 hours weekly. Musculoskeletal symptoms were reported by 64.5% (49/76). Burnout-related indicators included frequent fatigue (28.9%) and emotional exhaustion (17.1%). Working ≥41 hours per week was independently associated with frequent fatigue. A significant complication was reported by 64.5%, with psychological effects including sleep disturbance (56.6%), intrusive thoughts (27.6%), and loss of confidence (35.5%). Avoidance of similar procedures occurred in 35.5%, while 60.5% reported no formal debriefing or institutional support.
Conclusion:
Endoscopists in Saudi Arabia experience a substantial physical and psychological burden, compounded by limited institutional support. Targeted interventions, including ergonomic training, structured debriefing, and peer-support programs, are needed to improve clinician wellbeing and sustain endoscopy services.
