Abstract

Background:

Multiple organ failure (MOF), defined as the failure of at least two major organ systems, is a major complication in patients with systemic inflammatory response syndrome (SIRS). Postinjury MOF accounts for 51%–61% of late trauma-related deaths and is linked to sustained inflammation and immune dysregulation.

Objectives:

The objective of the study was to assess the association between post-traumatic SIRS and the development of MOF in patients admitted to the intensive care unit (ICU) at King Abdulaziz Medical City (KAMC), Riyadh and to identify independent predictors of MOF among ICU trauma patients.

Methods:

This retrospective cohort design included 277 trauma patients aged a14 years. Data were collected from the KAMC Trauma Registry and BestCare database. MOF was identified using the Sequential Organ Failure Assessment score, and SIRS was defined by standard clinical criteria. Chi-square and multivariable logistic regression were used ( P < 0.05).

Results:

Among 277 patients, 28.9% had no organ failure, 30.7% had single organ failure, and 40.4% had MOF. Mean age varied significantly across groups ( P < 0.001), but gender distribution did not ( P = 0.45). SIRS was present in 55% of patients without organ failure, 75.2% with single organ failure, and 79.4% with MOF ( P = 0.001). Blunt trauma was the most common injury type ( P = 0.044). The central nervous, cardiovascular, and respiratory systems were most frequently affected. Mortality rates were significantly higher in patients with MOF.

Conclusion:

In conclusion, our study found that there is an association between post-traumatic SIRS and the development of MOF in patients admitted to ICUs. Further study needs to be conducted regarding the type of intervention in different mechanisms of organ failure.
