SP 5.13 The Intercollegiate Green Theatre Checklist (v2): A Survey of Feasibility and Practicality

Introduction
The Green Theatre Checklist (GTC) provides a framework to reduce the environmental impact of surgery on the day. Adoption remains sporadic and challenging. This study from the Sustainability Quality Improvement Collaborative surveys the perception of version 2 of the GTC from centres around the UK.
Methods
A survey questionnaire was distributed to active healthcare professionals (HCPs) in surgery using Microsoft Forms in January 2026. A Likert scale was used to evaluate perceptions of individual checklist items for usefulness and feasibility together with overall free text comments.
Results
There were 72 responses from surgeons (69%, of which 28% were consultants), anaesthetists (24%, 71% consultants) and nursing & operating department practitioners (7%). The majority (88%) did not use the GTC in normal practice, and 44.4% who had not heard of the GTC. Checklist items providing autonomy to operational processes (such as powering off equipment or asking if equipment should be opened) scored higher than items changing clinical practice on the day (such as considering total intravenous anaesthesia). Free-text feedback was generally positive but there were concerns about the timing of its use, it’s length and effectiveness as a checklist. Despite this, 76% felt the GTC should be part of normal practice.
Conclusion
Most surgical HCPs surveyed are not using or are unaware of the GTC despite recognising its importance. Future work should stratify items into those for integration into established checklists to empower staff to make change, and those where change is only realistically possible at an organisational level.
