Aim
A Comparison of Postoperative Outcomes Between Cyanoacrylate Glue and Tacker Fixation in Laparoscopic TAPP Inguinal Hernia Repair
Methods
Eligible patients who met inclusion criteria were randomly assigned to one of two groups using Chitbox randomisation method.
Both groups underwent laparoscopic TAPP repair.
Group A (n = 35) & Group B (n=35), mesh fixation with tackers and cyanoacrylate glue respectively.
Duration: May 2023 to April 2024 (12 months) with 6 months follow-up.
Results
Postoperative pain scores were significantly higher in Group A on day 1 (5.3 ± 0.5 vs 3.38 ± 0.6; p < 0.0001) and day 3 (3.1 ± 0.4 vs 2.31 ± 0.47), with persistent significance on day 7 (p = 0.003) and day 30 (p = 0.02).
Postoperative complications occurred in 6 patients (17.14%) in Group A and 4 patients (11.43%) in Group B, including seroma, hematoma, and surgical site infection.
The prevalence of postoperative nausea, vomiting, abdominal distension, and fever was comparable between the groups, with no statistically significant difference (p = 0.998).
Katz Index of Independence in Activities of Daily Living scores demonstrated significantly greater dependence Group A for bathing (4.4 vs 4.0; p = 0.0012), dressing (4.7 vs 4.29; p = 0.0183), transferring (3.4 vs 2.77; p < 0.0001), with no significant differences in toileting (p = 0.0989), continence (p = 0.5476), or feeding (p = 0.7691).
Conclusion
Cyanoacrylate glue offers an effective substitute for tackers regarding mesh fixation, offering added benefits reduced postoperative pain, prompt return to regular daily activities with no significant difference in post-operative complications, hospital stay and recurrence.
