Safer care process for patients undergoing gastrostomy (PEG) in Malmö

Introduction
Gastrostomy is an effective and safe method for enteral nutrition in cases of long-term malnutrition or dysphagia. Certain factors are associated with an increased risk of complications when conducting a PEG. Responsibility for examination, decision-making and possible optimization lies with the referring physician. At the same time, there is generally low knowledge about gastrostomy and PEG. Patient safety deficiencies have been identified in the care for patients receiving a PEG in SUS Malmö. Therefore it is relevant implement a local Care Program for conducting PEG at SUS Malmö.
Methods
Needs analysis was carried out through a process meeting with representatives from the Surgery and Neurology departments. Patient safety risks were identified, and a decision was made to introduce a local Care Program for patients receiving a PEG. Planning and implementation then took place. Follow-up is planned for autumn.
Results
A local Care program was created based on current National and Regional guidelines and is being implemented for Malmö SUS. The care program creates an overview of the process. The care program also includes two patient brochures and a PEG form. The PEG-form purpose is to detect risk of complications. Training för the staff in the care department often meeting patients with a PEG has been held.
Discussion
Getting a clear overview of a care process is crucial to creating coherent, safe and effective care for the patient. By process-oriented care, the focus shifts from the performance of individual units to the patient's overall journey - from first contact and examination to treatment and follow-up.
