Diarrhea-associated hemolytic uremic syndrome (D + HUS) is common, particularly among infants. It is one of the common causes of community-acquired acute kidney injury in children and also leads to nonrenal complications. We hereby report the case of an infant with severe and refractory D + HUS, with neurologic involvement, in whom complement blockade with eculizumab provided a rescue therapy. To date, there is no specific therapy available for D + HUS, and the cornerstone of management is supportive care. In selected cases with severe disease and neurologic involvement, complement blockade may be considered a treatment option.
