Reconstruction of a distal toe defect using a cross-toe flap in a high-comorbid patient with post-traumatic subacute osteomyelitis: a case report

Rio Aditya, Fransisca Sisca, Andri Primadhi

Introduction:

Subacute osteomyelitis (SOM) following minor trauma is a rare clinical entity that presents significant reconstructive challenges, particularly in patients with comorbidities. The cross-toe flap is an underutilized but effective technique for distal toe salvage.

Case presentation:

A 62-year-old man with multiple comorbidities presented with sepsis and SOM of the third toe following a minor motorcycle accident. Treatment involved a multi-phase surgical approach, including a cross-toe flap. Despite initial flap necrosis, the formation of an underlying bio-membrane allowed for successful salvage with a full-thickness skin graft.

Conclusion:

The cross-toe flap remains a resilient tool for distal foot reconstruction, even when faced with partial failure in high-risk patients.
