Rare Co-occurrence of Two Distinct Mesenchymal Tumors: Lipoleiomyoma of the Uterus and Phyllodes Tumor of the Breast

BSTRACT

Uterine lipoleiomyomas are rare, benign tumor of smooth muscle cells admixed with mature adipocytes, which account for only 0.03%–0.2% of all uterine leiomyomas. Although generally small and without symptoms, they can at times become large abdominopelvic masses with clinically mimicking malignancy. We present a very uncommon case of a giant lipoleiomyoma of about 30 cm × 27 cm × 20 cm in a 46-year-old woman who was admitted with worsening abdominal distension, constipation, and weight loss. Radiological imaging was suspicious for uterine sarcoma based on size, vascularity, necrosis, and compression of the adjacent organs. Surgical resection and histopathological assessment, however, established a lipoleiomyoma. The patient also presented with a lump in the breast of size 5 cm × 4 cm × 3 cm which was suggested as benign phylloids tumor on radiology and later on confirmed the same on histopathology. This case emphasizes the need to combine imaging with histopathology for proper diagnosis and management.
