The defects in the iris tissue often result in glare, photophobia and impaired cosmesis hampering the routine activities of patients. Iris defects could be congenital or could be attributed to either blunt or penetrating trauma including the surgery induced damage resulting in iris tissue loss or iridodialysis. Various treatment options include colored contact lenses, corneal tattooing, iris prostheses, intraocular lens (IOL) with opaque borders and custom-made artificial iris. Pupillary reconstruction in case of healthy iris tissue with the use of iris sutures can be done in iris defects of up to around 2 clock hours; however, there are limited options currently available for the larger defects. Therefore, a customised surgical plan is required for every patient owing to the complexity and varied aetiology of the occurrence of iris defects. We report a novel technique of pupillary reconstruction using a PMMA iris-colored ring segment, which improves cosmesis and helps prevent secondary angle-closure glaucoma in selected patients.
