A 69-year-old retired engineer presents with a 3-year history of perceived parasitic infestation, describing crawling sensations and engaging in compulsive skin inspection and self-extraction behaviors. Examination reveals excoriations and ulcerations consistent with self-inflicted injury. Despite normal laboratory and neurological findings, his belief persists and is later shared by his wife, who develops similar symptoms and participates in the same behaviors, leading to social withdrawal.
