Aim: To investigate the association between fragmented QRS (fQRS) on electrocardiography and the Systemic Immune-Inflammation Index (SII) with disease severity in patients with acute pancreatitis (AP).Methods: This retrospective observational study included 92 patients diagnosed with AP between January 2022 and December 2024. Clinical, laboratory, and electrocardiographic data at admission were analyzed. SII was calculated as platelet count × (neutrophil/lymphocyte). Severe AP was defined as a BISAP score ≥3. Patients were compared according to the presence or absence of fQRS. Multivariable logistic regression analysis was conducted including age, SII, and fQRS. Receiver operating characteristic (ROC) analysis was used to evaluate the predictive performance of SII.Results: Fragmented QRS was detected in 18 patients (19.6%). Patients with fQRS had significantly higher SII values and BISAP scores compared with those without fQRS (p=0.020 and p
