Extrapulmonary tuberculosis (EPTB) represents 15%–20% of all tuberculosis (TB) cases. TB of soft-tissue abscesses is a rare form of EPTB, making up <1%–2% of cases. This report describes a 2-years-old boy who presented with a persistent and painless lump in right leg for 1 year. Medical history was normal, and had no known exposure to TB. A
Bacillus Calmette-Guérin
(BCG) scar was noted on right upper arm, and the tuberculin skin test indicated an induration of 15 mm. A chest X-ray revealed right hilar lymphadenopathy. A biopsy of the wound, examined with hematoxylin-eosin staining revealed chronic granulomatous inflammation characterized by multinucleated Langhans-type giant cells. Microbiological examination (GeneXpert
Mycobacterium tuberculosis
/rifampicin [MTB/RIF]) of the wound biopsy was positive, but Ziehl–Neelsen staining did not detect MTB. Anti-tuberculous therapy was initiated with favorable results after 6 months of TB treatment, and no side effects documented.
