Odontogenic keratocyst (OKC) is a benign but locally aggressive odontogenic lesion with a high recurrence rate. Surgical enucleation remains a commonly employed treatment modality; however, postoperative complications such as wound dehiscence may adversely affect healing and functional outcomes. This report describes the case of postoperative suture dehiscence following enucleation of an OKC in the anterior mandible, resulting in a through-and-through vestibular defect. Conservative management was carried out using sequential povidone-iodine and gentamicin dressings, followed by obturator placement for functional rehabilitation. The approach facilitated satisfactory healing while avoiding additional surgical intervention. This case highlights the effectiveness of conservative management combined with prosthetic rehabilitation in managing postoperative complications of OKC.
