A 45-year-old female with an acute onset multilobulated choroidal elevation in her right eye was evaluated to rule out choroidal granuloma. However, multimodal imaging was suggestive of right eye acute central serous chorioretinopathy (CSC) with choroidal elevation secondary to ‘posterior choroidal fluid loculation (PCFL).’ Complete resolution of choroidal elevation was noted with observation alone. Though PCFL is described to be a common finding in CSC, the choroidal elevation is a rare finding. Posterior choroidal fluid loculation should be considered as a possibility in cases presenting with choroidal elevation and signs of pachychoroid.
