Post-obstructed labor vaginal synechiae leading to sexual dysfunction: A case of successful reconstructive management

Postpartum vaginal synechiae are rare but distressing, leading to significant morbidity, including apareunia, menstrual abnormalities, and secondary infertility if left untreated. We report a 26-year-old primipara who developed distal circumferential vaginal synechiae after obstructed labor complicated by puerperal infection. She presented with progressive dyspareunia leading to apareunia along with changes in menstrual pattern. She was found to have a distal circumferential vaginal synechiae with only a pinhole-sized opening. Surgical adhesiolysis was done, followed by postoperative vaginal dilation, which restored vaginal patency and sexual function. This case underscores the importance of early recognition and prompt management of postpartum genital tract complications and demonstrates how careful surgical planning, patient education, and structured postoperative care can result in favorable outcomes.
