Physiotherapy and platelet-rich plasma in anterior cruciate ligament management: A clinical perspective on integrated rehabilitation

Abstract

Anterior cruciate ligament injuries continue to pose a substantial challenge in sports and orthopedic practice. While surgical reconstruction and physiotherapy remain central to treatment, biological adjuncts such as platelet-rich plasma (PRP) are increasingly considered. In my opinion, integrating PRP with physiotherapy holds significant promise for optimizing rehabilitation outcomes, yet its translation into routine clinical practice remains inconsistent. Evidence suggests that PRP’s bioactive properties may accelerate tissue healing and, when combined with structured physiotherapy, may reduce pain, improve functional outcomes, and support early pain reduction and functional progression during rehabilitation. However, inconsistencies in PRP preparation methods, lack of standardization in physiotherapy protocols, and limited high-quality trials continue to hinder the consensus. This paper argues that clinicians and researchers should not view PRP as a replacement but rather as a complementary tool within physiotherapy-centered rehabilitation. To maximize its impact, collaborative efforts among sports physicians, physiotherapists, and researchers are essential for establishing clear guidelines, ensuring equitable access, and focusing on long-term, patient-centered outcomes.
