Phthiriasis palpebrarum is an ectoparasitosis of the eyelids caused by Phthirus pubis (crab lice). Due to its relative rarity and similar symptoms, it may be misdiagnosed as demodex blepharitis. We present the case of a 9-year-old girl presenting with phthiriasis palpebrum successfully treated with mechanical removal followed by a combination of topical antibiotics and 7% tea tree oil, with no recurrence at 2 months of follow-up.
