P138 Retrospective real-world evaluation of Wynzora cream efficacy and safety

Abstract

Plaque psoriasis is a chronic immune-mediated inflammatory skin disease affecting 2–3% of the UK population and is associated with significant physical and psychosocial burden. For patients with mild-to-moderate disease, topical therapy remains the cornerstone of management, although adherence is often limited by the poor cosmetic acceptability of traditional formulations. Fixed-dose calcipotriol–betamethasone dipropionate (CAL/BDP) combinations are recommended first line due to once-daily dosing and favourable safety. Wynzora® cream (CAL/BDP PAD cream) utilizes PolyAphron Dispersion (PAD) technology to enhance stability, penetration and patient acceptability. While phase III trials have demonstrated robust efficacy and quality-of-life benefits, UK real-world evidence remains limited. We conducted a retrospective real-world evaluation of all patients prescribed Wynzora cream across two district general hospitals. Demographic, clinical and patient-reported outcomes were assessed at baseline, 3 months and 6 months. These included Psoriasis Area and Severity Index (PASI), Physician’s Global Assessment (PGA), body surface area (BSA) and Dermatology Life Quality Index (DLQI). Patients without ≥ 3 months of follow-up were excluded. Twenty-seven patients were included (median age 34 years), representing an ethnically diverse cohort. Baseline disease burden was moderate (median PASI 3.6; median BSA 12.8%), with 70% receiving concurrent systemic immunosuppressive therapy, reflecting a complex, treatment-refractory population. By 6 months, PASI reduction was statistically significant (P = 0.02), with DLQI improvement trending towards significance. Among patients with evaluable 6-month PGA data (n = 16), seven (44%) achieved treatment success (PGA clear or almost clear or ≥ 75% improvement in PASI), including complete clearance in three patients. The real-world treatment success rate was comparable with that in pooled phase III trial data (43.2%), despite inclusion of patients on systemic therapies typically excluded from trials. High patient satisfaction was driven by the nongreasy, easy-to-apply formulation, with particular benefit reported for scaling and scalp involvement. Wynzora cream demonstrates meaningful real-world efficacy, safety and acceptability, supporting its role as both primary and adjunctive topical therapy in routine clinical practice.
