Introduction
Type 1 diabetes (T1D) requires continuous support to prevent acute complications such as hypoglycemia and diabetic ketoacidosis. In low- and middle-income countries, access to real-time diabetes guidance remains limited.
Objectives
To evaluate the utilization patterns and early outcomes of a 24/7 diabetes helpline for individuals with T1D in a resource-limited setting.
Methods
This prospective observational study was conducted between Aug 2025 and March 2026 at the Diabetic Association of Pakistan. A 24/7 telephonic helpline was established for individuals with T1D. Trained diabetes educators, under physician supervision, provided real-time guidance for insulin adjustment, glycemic fluctuations, and general diabetes care. Data were recorded through a call management system.
Results
A total of 202 calls from 88 individuals were recorded. Females comprised 66.3%. Mean age was 23.9 years, mean diabetes duration 10 years, and mean HbA1c 9.9%. Most calls involved insulin dose adjustment (24.3%), followed by hyperglycemia (17.6%). Nearly 50% of calls occurred during late evening hours. Only three cases required referral to healthcare facilities.
Conclusions
A 24/7 diabetes helpline is feasible and potentially cost-effective in resource-limited settings. Most concerns can be managed remotely, highlighting the importance of real-time support in T1D care.
