Abstract

Introduction

Continuous glucose monitoring (CGM) has revolutionized the management of type 1 diabetes mellitus (T1D), enabling more precise glycemic control. Unlike capillary glucose measurement using glucometers, CGM provides real-time data and advanced metrics such as time in range (TIR) and glycemic variability. Numerous studies have demonstrated that its use improves metabolic control, reducing glycated hemoglobin (Hba1c) levels and the incidence of ketoacidosis

Objectives

To evaluate the impact of treatment with CGM system on metabolic control in patients with 1 DM

Methods

Prospective observational study. Inclusion criteria: Patients aged 6 to 18 years with a diagnosis of type 1 DM with >1 year of disease duration, who initiate treatment with a CGM system for a period of 3 months. Outcome Variables: Changes in Hba1c, glucose management indicator (GMI), time in target range (TIR), time below range level 1 (TBR1), time below range level 2 (TBR2), time above range level 1 (TAR 1), time above range level 2 (TAR 2) and coefficient of variation (CV)

Results

A total of 23 patients were enrolled in the study (52% female, n=12), with a mean age of 11.6 years (range: 6.69–18.04) and a mean diabetes duration of 4.7 years (range: 1.21–12.21). Regarding glycemic outcomes, the mean GMI significantly decreased from 8.15% at baseline to 7.75% at the 3-month follow-up (p=0.032). Similarly, a significant reduction was observed in TBR <54 mg/dL (TBR 2), which declined from a baseline mean of 0.83% to 0.458% at the end of the study period (p=0.049)

Conclusions

CGM compared with self-monitoring of blood glucose (SMBG), has been shown to reduce both GMI levels and the frequency of severe hypoglycemic events in patients with 1 DM
