P-157: Telemedicine-enabled dietary and behavioral follow-up after bariatric surgery in obese children with type 1 diabetes: a digital care model for postoperative optimization
V Sharma, S SharmaAbstract
Introduction
Postoperative management plays a critical role in determining outcomes after bariatric surgery, particularly in obese children with type 1 diabetes (T1D), who are prone to sleep disturbances and anxiety that can impair recovery and glycemic control
Objectives
To assess the effectiveness of a telemedicine-enabled dietary and behavioral follow-up model in improving postoperative outcomes in this population
Methods
A total of 380 children with T1D undergoing minimally invasive bariatric surgery were included. Patients with sleep and anxiety disorders were assigned to either a telemedicine follow-up group (n=258) or standard outpatient care (n=122). The intervention delivered structured nutritional guidance, sleep hygiene education, and behavioral strategies via a digital platform. Outcomes were evaluated at 1, 3, and 6 months.
Results
The telemedicine group demonstrated significantly better adherence to dietary recommendations and improved sleep patterns compared to standard care. At 6 months, 66.4% achieved at least 50% reduction in sleep and anxiety symptoms, while 33.7% showed improvements exceeding 80%. Additional benefits included improved emotional regulation, reduced irritability, and better family routine organization. Caregiver satisfaction was high (92.8%), driven by convenience and accessibility
Conclusions
Telemedicine-based follow-up is a feasible, scalable, and effective approach for optimizing postoperative recovery in pediatric bariatric patients with T1D. It enhances adherence, supports behavioral stability, and provides a practical solution for long-term multidisciplinary care
