Osteoid Osteoma of the Body of Cervical Vertebra—A Case Report of an Unusual Presentation

Abstract

Osteoid osteoma (OO) is a benign bone forming tumor. The incidence of Spinal OO is 10-25%, common being in the lumbar region, rarely occurs in cervical spine (4% of cases). It is usually seen in the posterior elements. Here is rare case of OO of the C4 vertebral body, presenting with restriction of neck movements, which was suspected to be infection. Bone scan scintigraphy and CT scan revealed the OO occupying posterior and inferior aspect of C4 body, in close proximity to the cord. Hence radiofrequency ablation was precluded with risk of thermal injury to neural structures. Since the inferior end plate was breached, the tumor was approached via C4-5 anterior discectomy. Curettage of lesion and fusion was done. Histopathology confirmed the diagnosis. High index of suspicion, detailed assessment and proper planning are critical in diagnosing and managing this rare tumor.
