Ocular surface squamous neoplasia (OSSN) is the most commonly misdiagnosed ocular surface lesion. We report a unique case of a 65-year-old female patient, presenting with constant itching, localized redness, and watering for 1 year in LE. She had a recent onset gradually progressive painless blackish discoloration over the temporal quadrant of the LE for 1 month. It was initially misdiagnosed as necrotizing scleritis, but after excisional biopsy, the diagnosis of OSSN was confirmed. Thus, it is imperative on the part of the ophthalmologist to have a strong clinical suspicion in treatment of recalcitrant cases to rule out the possibility of OSSN.
