Background
Artificial intelligence-assisted surgery is advancing rapidly, offering potential to enhance intraoperative planning and execution for improved outcomes. However, its application in open heart surgery remains limited. We have developed the Multi-channel Intraoperative Robotic Assisted and Collaborative Quality-control (MIRACQ) system, which overcomes challenges including intelligent surgical field tracking and multi-source data fusion, enabling "holographic" recording of surgical procedures with real-time artificial intelligence assistance.
Purpose
This study aimed to evaluate the clinical efficacy of the MIRACQ system in patients undergoing coronary artery bypass grafting, with a focus on major adverse cardiovascular and cerebrovascular events and perioperative outcomes.
Methods
Patients who underwent coronary artery bypass grafting between January 2022 and September 2023 were retrospectively enrolled and divided into groups according to whether the MIRACQ system was used intraoperatively. The association between MIRACQ use and in-hospital outcomes and perioperative parameters was evaluated.
Results
A total of 2040 patients were included, comprising 514 in the MIRACQ-assisted group and 1526 in the control group. Multivariable logistic regression analysis demonstrated that MIRACQ use was associated with reduced risks of in-hospital MACCE (OR=0.424, 95%CI: 0.192-0.839, P=0.021) and stroke (OR=0.056, 95%CI: 0.003-0.263, P<0.01). After propensity score matching, 506 pairs were formed. The MIRACQ-assisted group showed significantly lower rates of MACCE (1.8% vs 4.5%, P=0.012) and stroke (0.2% vs 3.8%, P<0.01) compared with the control group. Logistic regression analysis confirmed that MIRACQ use remained associated with reduced risks of MACCE (OR=0.364, 95%CI: 0.162-0.817, P=0.014) and stroke (OR=0.053, 95%CI: 0.007-0.393, P<0.01).
Conclusion
Application of the Multi-channel Intraoperative Robotic Assisted and Collaborative Quality-control system in coronary artery bypass grafting is associated with reduced risks of in-hospital MACCE and stroke, suggesting its potential value in optimizing intraoperative quality control and team collaboration.Figure 1.Structure of the MIRACQ system.Figure 2.Intraoperative view during CABG.
