Methodological Aspects of Autotransplantation of Two or More Free Flaps in Head and Neck Organ Reconstruction. Development of a Clinical Algorithm

Introduction. The reconstruction of extensive composite defects of the maxillofacial region after oncological resections is one of the most challenging tasks in microsurgery. When adequate reconstruction with a single graft is not possible, multiflap reconstruction becomes the method of choice. The absence of unified standards for selecting the strategy and determining the sequence of reconstruction stages necessitates the development of a comprehensive algorithm. Aim. To develop an algorithm for selecting the strategy and planning the stages of reconstruction using two or more free flaps. Material and methods. A narrative review was conducted using the PubMed and Scopus databases for the period 2015–2025, together with a retrospective cohort study of 32 patients with malignant head and neck tumors who underwent reconstruction using 66 free flaps between 1993 and 2025, including 26 sequential and 6 single-stage reconstructions. Results. An algorithm was developed that includes: 1) risk stratification; 2) determination of the sequence of anatomical structure restoration in multistage treatment. Conclusion. Complex reconstruction using multiple free flaps is an effective method for closing extensive composite defects. The proposed algorithm standardizes surgical planning; however, it requires further prospective validation.

Введение. Устранение обширных сочетанных дефектов челюстно-лицевой области после онкологических резекций является одной из наиболее сложных задач в микрохирургии. При невозможности адекватного восстановления одним трансплантатом методом выбора становится мультилоскутная реконструкция. Отсутствие единых стандартов выбора стратегии и очередности этапов реконструкции определяет необходимость создания комплексного алгоритма. Цель. Разработать алгоритм выбора стратегии и планирования этапов реконструкции двумя и более свободными лоскутами. Материал и методы. Проведен нарративный обзор (базы PubMed, Scopus 2015–2025) и ретроспективное когортное исследование 32 пациентов со злокачественными новообразованиями головы и шеи, которым в период 1993–2025 гг. была выполнена реконструкция с использованием 66 свободных лоскутов (26 последовательных и 6 одномоментных реконструкций). Результаты. Сформирован алгоритм, включающий: 1) стратификацию рисков; 2) определение последовательности восстановления анатомических структур при многоэтапном лечении. Заключение. Комплексная реконструкция несколькими свободными лоскутами является эффективным методом закрытия обширных комбинированных дефектов. Предложенный алгоритм стандартизирует хирургическое планирование, однако требует дальнейшей проспективной валидации.
