Minimally invasive surgery (MIS) bunion correction has been increasingly popular among foot and ankle surgeons due to its proven efficacy and safety. At this juncture, we need to continue evolving more sophisticated techniques to produce better outcomes with fewer complications. The MIS burr is a very useful and safe tool to cut the hallux metatarsal at its epiphyseal junction when used with appropriate caution. However, regarding excision of the proximal fragment medial spike and medial eminence of the metatarsal head, it is relatively harder to adequately protect the soft tissues that lie just on the surface of the bone that we aim to remove. Here, we present a novel method of excising the medial eminence by the utilization of a commercially available MIS bunion parallel guide and the burr.
