Medial patellofemoral ligament (MPFL) reconstruction is a mainstream surgical approach for treating recurrent patellar dislocation. In recent years, double‐bundle reconstruction, for example, combined reconstruction of MPFL and the medial quadriceps tendon‐femoral ligament, has been applied and shown favorable outcomes in the treatment of recurrent patellar dislocation. Herein, we describe an alternative surgical strategy whereby medial quadriceps tendon‐femoral ligament is replaced by medial patellar retinaculum cable, combined with MPFL reconstruction and medial tightening. The 1‐cm‐width longitudinal retinaculum cable along the medial patella is dissected from the distal end while leaving the proximal attachment to quadriceps tendon insertion intact. The harvested autograft tendon for MPFL reconstruction is secured together with the free end of the medial patellar retinaculum cable. The vastus medialis oblique and the remaining medial patellar retinaculum are fastened onto the patella. This technique presents a readily adoptable and effective surgical approach for treating recurrent patellar dislocation.
