Maxillary sinus micribiology across different clinical conditions: a systematic review and meta-analysis of culture-based and molecular studies

Background: The microbiology of the maxillary sinus varies according to clinical phenotype, sampling technique, and detection method. This study aimed to systematically synthesize culture-based and molecular evidence on maxillary sinus microbiology in clinically non-inflamed sinuses, chronic rhinosinusitis (CRS), and odontogenic sinusitis (OS). Material and methods: A systematic search of PubMed was conducted, followed by a quantitative synthesis of studies in which specimens were obtained directly from the maxillary sinus. Pooled estimates were calculated using a binomiallogit random-effects model. Results: In clinically non-inflamed maxillary sinuses, the pooled prevalence of positive bacterial cultures was 48.4% (95% CI 15.9–82.3%; k=8). Among patients with CRS, the pooled prevalence of anaerobic bacterial detection was 36.9% (95% CI 10.2–75.1%; k=7). Substantial between-study heterogeneity was observed. In OS, both culture-based and molecular studies demonstrated a predominance of polymicrobial communities of oral origin. Conclusions: Microbiological findings from the maxillary sinus should be interpreted in the context of the clinical phenotype, anatomical sampling site, and detection method.
