Mauriac syndrome as a complication of uncontrolled type 1 diabetes mellitus: A case report from a limited health facility

Mauriac syndrome is a rare complication of poorly controlled type 1 diabetes mellitus (T1DM). Diagnosis is challenging in resource-limited settings where definitive tools like liver biopsy are unavailable. An 11-year-old boy with T1DM and chronic poor glycaemic control presented with growth failure, abdominal distension and transaminases > 10 times the upper normal limit. Due to the lack of HbA1c testing and imaging, Mauriac syndrome was suspected clinically. Insulin regimen was intensified using a basal long-acting mix. Eight weeks later, glycaemic control improved, liver span decreased from 10 cm to 8 cm and transaminase levels significantly decreased. A therapeutic trial can help confirm the diagnosis in limited settings, as the condition is reversible with proper insulin therapy.
