Massive Hemoptysis in an 86-year Old – A Case of Bronchial Artery Pulmonary Vein Fistula Unmasked
C. Yokesh Kanna, Gayathri Devi Anur RamakrishnanAbstract
Massive hemoptysis is a life-threatening event that requires urgent identification and intervention. We report the case of an 86-year-old male who presented to our hospital with a bout of massive hemoptysis and hemodynamic instability. He was initially stabilized and managed conservatively, but proceeded to have another episode of hemoptysis. An urgent flexible fiberoptic bronchoscopy was done to help localize the bleed, and the patient was taken up for bronchial artery embolization (BAE), following which the bleed stopped. This case highlights an unusual fistulous communication between the bronchial artery and pulmonary vein as the source of massive hemoptysis, underscoring the importance of thorough imaging and BAE in such cases.
