Gingival recession, defined as apical migration of the gingival margin, presents aesthetic concerns and risks such as root sensitivity and root caries. This case report describes the successful management of localised gingival recession using a gingival unit graft (GUG). A 21-year-old female presented with a chief complaint of receding gums in the mandibular anterior region. Clinical examination revealed a Miller Class Ⅰ gingival recession associated with inadequate keratinised tissue. Treatment involved harvesting a GUG from the palatal donor site and adapting it precisely to the recipient area. Post-operative evaluations at 2 weeks, 3 months, 6 months and 1 year demonstrated excellent root coverage, increased gingival thickness and satisfactory aesthetic outcomes. No complications such as graft failure or infection were observed and the graft showed good integration with adjacent tissues. This case highlights the GUG as a predictable and effective option for managing gingival recession when proper case selection, surgical technique and post-operative care are followed.
