Lower Periorbital Rejuvenation Using Combined High-intensity Facial Electrical Stimulation and Synchronised Radiofrequency: A Case Series

Joyce Teng-Ee Lim

Lower periorbital ageing is a multifactorial process involving skin laxity, weakening of the orbicularis oculi muscle, fat herniation, and impaired lymphatic drainage, and clinically presenting as eye bags, wrinkles, and dark circles. Although surgical blepharoplasty remains effective, many patients prefer non-invasive treatments with minimal downtime. A novel device that combines synchronised radiofrequency (RF) and high-intensity facial electrical stimulation (HIFES™) enables the simultaneous treatment of the dermal and muscular components of ageing. This case series evaluated the efficacy and safety of combined synchronised RF and HIFES™ for lower eyelid rejuvenation in Chinese patients. Eleven Chinese patients (10 females, 1 male; aged 42-70 years) with mild-to-severe lower periorbital ageing were recruited. Each patient underwent 6 weekly 20-minute EMFACE → Eye treatment sessions. Clinical assessments were performed at baseline, at 1 month, and 2 months (weeks 9 and 13) after the final treatment. The assessment included improvement in eye bags, skin texture, wrinkles, pigmentation, and patient satisfaction. Standardised photographs were obtained using a Quantificare imaging system, and patients’ satisfaction and adverse events were recorded. All patients demonstrated visible improvements in the appearance of the lower eyelids. Eye bag prominence was reduced across all severity grades, from mild to severe cases. Improvements in skin texture, smoothness, fine wrinkles, and dark circles were observed, with further enhancement at 2-month follow-up, suggesting ongoing collagen remodelling and muscle adaptation. Puffy eyes improved in patients with lymphatic congestion. No pain, downtime, or treatment-related adverse effects were reported. Patient satisfaction was uniformly high, with patients reporting a natural, refreshed appearance. Combined synchronised RF and HIFES™ (EMFACE→) treatment is a safe, well-tolerated, and effective non-invasive option for lower periorbital rejuvenation in Chinese patients. This may be particularly beneficial for patients seeking natural results without surgery or downtime.
