Low-dose, High Impact: Pregabalin-induced Peripheral Edema in the Elderly

Pregabalin is widely used for neuropathic pain, including in elderly patients. Peripheral edema is a known side effect, typically associated with higher doses. However, clinically significant edema at lower doses is rarely reported. We present the case of a 69-year-old woman who developed bilateral lower limb edema after 7 months of therapy with low-dose pregabalin (50 mg/day). There were no comorbidities or other causes for edema. After discontinuation of pregabalin, the edema resolved significantly within 2 weeks. Even low-dose pregabalin can cause peripheral edema in elderly patients. Clinicians should be cautious with its use in geriatric populations and remain alert to subtle adverse effects.
