Leash effect of bilateral superior oblique muscle in Weill–Marchesani syndrome

Varsha Tamrakar, C S Lalitha, Lakshmi Nayak, Archita Das

Weill–Marchesani syndrome (WMS) is a rare connective tissue disorder characterized by short stature, brachydactyly, and distinctive ocular abnormalities such as microspherophakia. We report a 23-year-old male with classical systemic and ocular features of WMS associated with a leash effect of bilateral superior oblique muscle (Brown syndrome). Extraocular motility abnormalities have not been classically described in WMS. The coexistence of bilateral Brown syndrome in our patient raises the possibility that the underlying connective tissue defect in WMS may also involve the extraocular muscle–tendon complex, thereby expanding its recognized ophthalmic spectrum.
