Laparoscopic Transmesocolic Excision of Large Paraganglioma in Aortocaval Window: A Rare Case Report and Review of the Literature

Abstract

Paragangliomas (PGLs) are rare extra-adrenal pheochromocytomas situated around the bifurcation of the aorta, arising from chromaffin cells. Commonly presenting with hypertension like adrenal pheochromocytoma, biochemical investigations, such as serum catecholamines along with contrast-enhanced computerized tomography, are required to confirm the diagnosis. Juxtaposition with the aorta and inferior vena cava poses a diagnostic and surgical challenge. Being retroperitoneal, it is approached by reflecting the colon. We report a case of transmesocolic excision of a large PGL with an approach through the mesocolon.
