Kidney Transplantation into a Continent Ileocecal Reservoir (Penn Pouch): Technical Considerations in a Complex Case

Kidney transplantation in patients with urinary tract reconstruction is surgically challenging. We report a case of a 26-year-old female who underwent successful deceased donor kidney transplantation using a preexisting continent ileocecal reservoir (Penn pouch). The patient had undergone urinary diversion in early childhood for congenital bladder agenesis, solitary kidney, and ectopic ureter. Detailed pretransplant evaluation and tailored surgical planning enabled successful graft implantation and uretero-pouch anastomosis along the taenia coli.
