Isolated unilateral congenital knee dislocation in a neonate

ABSTRACT

Congenital dislocation of the knee (CDK) is a rare orthopaedic condition identified at birth, characterised by hyperextension and anterior displacement of the tibia relative to the femur. We present the case of a term neonate with a hyper-extended and anteriorly dislocated right knee, observed immediately after birth. Clinical evaluation and radiographic imaging confirmed the diagnosis of an isolated CDK. A conservative treatment approach was undertaken using serial above-knee casting, with the knee maintained in maximal flexion. The casts were changed weekly for 5 weeks, resulting in gradual and complete correction of the deformity. This case highlights the importance of early recognition and timely conservative management in achieving successful outcomes in isolated congenital knee dislocation, potentially avoiding the need for surgical intervention.
