Aims: To evaluate CURB-65 score in community-acquired pneumonia (CAP) with nutritional parameters and to compare the discriminative performance of modified scores created by adding serum albumin or the Prognostic Nutritional Index (PNI) (A-CURB65 and CURB65-PNI) with the conventional CURB-65.Methods: A total of 191 patients hospitalized with CAP between January 2024 and March 2026 were retrospectively analyzed. The A-CURB65 model, based on albumin levels, and the CURB65-PNI model, based on PNI, were developed. Models were evaluated using receiver operating characteristic (ROC)/area under the curve (AUC) analysis, the DeLong test and reclassification analyses. The 95% confidence interval (CI) for the difference in AUC between A-CURB65 and conventional CURB-65 was estimated using 10,000 bootstrap resamples. Results: Of the 191 patients, 25 (13.1%) died. In parsimonious multivariable models adjusted for continuous CURB-65, albumin [odds ratio (OR) per 1-g/dl increase: 0.06; 95% CI: 0.02-0.19; p
