Editorial Commentary : Hip Arthroscopy After 40 Years of Age: The Joint Matters More Than the Birth Certificate

Abstract

Hip arthroscopy after 40 years of age remains controversial in hip preservation surgery. Carefully selected patients can still achieve meaningful clinical improvement; however, survivorship remains inferior compared with younger populations. Chronological age alone should not dictate surgical indications, as the biologic status of the joint likely matters far more than the birth certificate itself. Cartilage integrity, degeneration, and joint preservation remain critical determinants of outcomes. Although modern techniques have improved hip preservation surgery considerably, technical evolution cannot fully overcome advanced degeneration. Ultimately, symptom improvement does not necessarily equal long‐term survivorship, and careful patient selection remains everything.
