Abiotrophia defectiva Endophthalmitis Associated With Exposed XEN Gel Stent

An 80-year-old woman presented with a painful red eye four months after XEN gel stent implantation for primary open-angle glaucoma. Examination revealed an exposed gel stent through a conjunctival defect, corneal edema, hypopyon, and vitreous membranes, consistent with acute exogenous endophthalmitis. She underwent vitreous aspiration with intravitreal vancomycin and ceftazidime, followed by pars plana vitrectomy with stent explantation and silicone oil tamponade. Vitreous cultures grew
