Sweet syndrome (SS) is an uncommon neutrophilic dermatosis associated with a number of systemic conditions including infections, inflammatory bowel disease and hematologic malignancy. Histiocytoid Sweet syndrome is a histologic variant of SS in which the dermal infiltrate contains histiocyte-like immature myeloid cells. This variant may confer an increased risk of association with hematologic malignancies. In patients with darker skin types, SS may be misdiagnosed, in part due to a lack of representative images in the literature. Misdiagnosis may prevent patients from obtaining the necessary workup for underlying conditions. Herein we present a case of a 41-year-old woman with Fitzpatrick type VI skin diagnosed with histiocytoid sweet syndrome.
