With the innovation and progressive maturation of arthroscopic techniques, the advantages of minimally invasive surgery and accelerated rehabilitation have made arthroscopy a preferred option for patients with rotator cuff injuries. However, for complex rotator cuff tears, such as “L‐shaped” tendon tears, the risk of postoperative retear remains considerable. The cable structure, serving as a “suspension bridge” within the rotator cuff, is a critical load‐bearing component of the shoulder joint. This structure is frequently compromised in “L‐shaped” rotator cuff tears. In this report, we describe an arthroscopic technique using high‐strength tape to restore the cable structure and treat “L‐shaped” rotator cuff tears. The key steps include using high‐strength tape to reduce the retracted tendon stump and restore the cable structure, thereby re‐establishing mechanical integrity, reducing tension on the repaired tendon, and minimizing the cutting effect of sutures on the tendon. Finally, the sutures from the medial‐row anchor are employed to perform a “Margin Convergence” repair of the longitudinal tear component.
