Higher LVEF associates with lower eGFR in a single-centre cardiorenal clinic: a cross-sectional analysis

Background/Introduction
The relationship between left-ventricular systolic function and renal function in routine cardiorenal practice is incompletely characterised.
Purpose
To quantify the association between left-ventricular ejection fraction (LVEF) and estimated glomerular filtration rate (eGFR) in a single-centre cardiorenal clinic (patients attended January–December 2025), and to compare eGFR across guideline-defined LVEF phenotypes (HFrEF <40%, HFmrEF 40–49%, HFpEF ≥50%).
Methods
Single-centre, cross-sectional analysis of patients attended between January 2025 and December 2025 in a prospectively maintained cardiorenal clinic. Variables were standardised to a data dictionary with range checks and cross-field logic; core fields showed ≥85% completeness (e.g., LVEF 47/52). Analyses used complete eGFR–LVEF pairs recorded in the registry. Primary endpoint: correlation between eGFR and LVEF (Pearson and Spearman). Secondary endpoint: eGFR (median [IQR]) across LVEF phenotypes (HFrEF <40%, HFmrEF 40–49%, HFpEF ≥50%). Database N = 52; complete eGFR–LVEF pairs n = 47.
Results
Among 47 complete pairs, eGFR correlated inversely with LVEF (Pearson r = −0.359, p ≈ 0.012; Spearman ρ = −0.332, p ≈ 0.023). A graded pattern consistent with lower eGFR at higher LVEF (HFpEF) was observed; phenotype-level medians and interquartile ranges are shown in the accompanying table.
Conclusion(s)
In this real-world cardiorenal unit, higher LVEF was associated with worse renal function. These hypothesis-generating findings warrant validation in larger cohorts with longitudinal outcomes and may reflect referral patterns, congestion phenotypes or treatment effects.Figure 1For image description, please refer to the figure legend and surrounding text.Table 1 For image description, please refer to the figure legend and surrounding text.
