Health-related quality of life differences across heart failure categories

Abstract

Aim

This study aimed to determine the differences in health-related quality of life (HRQoL) among patients with heart failure (HF) with reduced, mildly reduced, preserved, and improved ejection fraction (EF) (HFrEF, HFmrEF, HFpEF, and HFimpEF).

Methods

Data on HRQoL were collected from the Türkiye Heart Failure (TURK-HF) Registry using the EQ-5D-5L instrument, which comprises a short descriptive system questionnaire (mobility, self-care, usual activities, pain or discomfort, and anxiety or depression) and a visual analog scale (EQ VAS).

Results

A total of 978 patients completed the EQ-5D-5L questionnaire and were included in the final analysis. Of those, 54.7% had HFrEF, 14.1% HFmrEF, 27.3% HFrEF, and 3.9% HFimpEF. Moderate-to-severe problems in mobility, self-care, usual activities, and pain or discomfort were significantly higher in patients with HFpEF and HFrEF than in those with HFmrEF and HFimpEF (Figure 1). In contrast, no differences in anxiety or depression were observed between the HF categories. The median EQ-5D index value was higher in patients with HFmrEF and HFimpEF than in those with HFrEF and HFpEF (0.82 and 0.80 versus 0.73 and 0.67, respectively; P < 0.001) (Table). A total of 653 patients completed the visual analog scale (EQ-VAS). The median EQ VAS score was higher in patients with HFmrEF and HFimpEF than in those with HFrEF and HFpEF (80 and 75 versus 70 and 65, P = 0.002, respectively) (Figure 2).

Conclusion

This analysis from the TURK-HF Registry suggest that patients with HFpEF and HFrEF had worse HRQoL measures than those with HFmrEF and HFimpEF.Figure 1For image description, please refer to the figure legend and surrounding text.Figure 2For image description, please refer to the figure legend and surrounding text.
