Glandular Lesions of the Urinary Bladder: Diagnostic Spectrum and Clinical Relevance – Case-based Insights

Abstract

A wide spectrum of glandular and pseudoglandular lesions can be encountered in the urinary bladder, ranging from entirely benign reactive processes to aggressive malignant tumors. These lesions may arise as part of a chronic inflammatory or metaplastic response of the urothelium or represent true neoplastic proliferations. Distinguishing between these various entities is crucial, as their clinical behavior, treatment strategies, and prognostic implications differ significantly. We report four of such cases. All the patients were presented with complaints of painless hematuria. Necessary clinical, radiological, and cystoscopic evaluations were done. Biopsies were sent for histopathological examination. Histological examination with immunohistochemistry analysis was done to reach the final diagnosis for each case. This series highlights the diagnostic spectrum of glandular lesions in the urinary bladder, emphasizing the role of histomorphology combined with a targeted immunohistochemical panel for accurate categorization. Early recognition, particularly of premalignant and malignant glandular lesions, is vital for optimal patient management and improved outcomes. Awareness of this spectrum ensures timely recognition of potentially aggressive lesions and facilitates optimal patient care.
